What they do

PACE stands for Program of All-Inclusive Care for the Elderly. Rather than a single service, it wraps a whole care team around an older adult — doctors, nurses, therapists, transport, meals, a day center and home care — with the goal of keeping them safely in their own home instead of a nursing facility.

Who qualifies

Age 55 or older; live within the PACE of West Baltimore service area (17 West Baltimore zip codes); qualify for nursing-home level of care; and be able to live safely in the community with PACE support.
