Before you go
Call first to find out whether you qualify — eligibility depends on a care-level assessment, not just age.
What they do
PACE stands for Program of All-Inclusive Care for the Elderly. Rather than a single service, it wraps a whole care team around an older adult — doctors, nurses, therapists, transport, meals, a day center and home care — with the goal of keeping them safely in their own home instead of a nursing facility.
Services offered
- All primary and specialty physician services
- Nursing services and regular needs assessments
- Transportation to appointments and the day center
- Rehabilitation and speech therapy
- Durable medical equipment
- Meals and nutritional counseling
- Recreational therapy and social engagement at the day center
- Personal and home care services
- Hearing, dental, vision and podiatry
- Pharmacy services and medication management
- Behavioral health support
- Respite and end-of-life care
Who qualifies
Age 55 or older; live within the PACE of West Baltimore service area (17 West Baltimore zip codes); qualify for nursing-home level of care; and be able to live safely in the community with PACE support.
